Committed to providing you with the most comprehensive, compassionate care, we need
you to bring the following information on your first visit, if possible:
- All related medical records, pathology reports, laboratory
reports, x-rays and diagnostic test reports. At the Cancer Care Center we work in
a team effort with your referring physician. This information is necessary to provide
comprehensive care. Please bring the above information with you or make arrangements
with your primary care physician to have your medical records provided to us in advance
of your first appointment. It would help us if you ask your primary care physician
to fax records to our office at (941) 748-7878.
- Be sure to bring proof of insurance and present it to the
receptionist. If you have supplemental insurance, please also submit verification
to the receptionist. For your convenience, we will file all primary and secondary
insurance claims.
- Make a list of any questions you may have for your physician.
Many times patients forget to ask those questions which are of most concern. Making
notes will help us to provide you with all the answers regarding your care.
